NDIS Continence Management Plans Made Simple

At Caremmunity, we develop NDIS-compliant Continence Management Plans that are practical, easy to follow, and clinically sound. Each plan is created by a qualified Continence Nurse Specialist and tailored to the Participant’s individual needs, goals, and daily routines.

Whether it’s used by carers, support workers, or the Participant themselves, our plans are designed to promote confidence, dignity, and continuity of care.

Our Continence Management Plans provide clear, personalised guidance for NDIS Participants living with bladder or bowel concerns. Whether a Participant is newly funded or requires an updated plan, Caremmunity offers nurse-led support to help manage continence needs at home, in the community, or during personal care.

We work closely with Support Coordinators, families, and Participants to ensure every plan meets clinical requirements, NDIS expectations, and the Participant’s goals. Plans are developed and outlined in the Participant’s report following a continence assessment.

What is an NDIS Continence Management Plan?

A Continence Management Plan is a detailed, clinical document created by a qualified Continence Nurse Specialist. This includes recommendations for continence products, step-by-step care routines, and practical strategies tailored to the individual’s goals, preferences, and living situation.

It’s designed to help Participants manage their continence needs effectively, while also supporting carers and support workers with clear guidance.

Continence Management Plans are created for the Participant, but they’re also a vital resource for support workers, carers, families, and even plan managers. They provide consistent guidance on product use, routines, and respectful and effective continence care—especially helpful when multiple people are involved in care delivery.

By having a structured plan in place, everyone on the care team can feel confident and clear in their role.

We ensure all clinical information is presented in a way that’s accessible for both Participants and professionals, helping streamline communication with the NDIS.

A Continence Assessment is a nurse-led clinical evaluation with the Participant or their representative that investigates bladder and bowel function, medical history, and lifestyle factors. This assessment helps identify continence needs and goals.

A Management Plan is the written report that outlines how to manage those needs day-to-day. It includes routines, product recommendations, costings and care strategies.

An assessment must occur for the Management plan and report to be written. Think of it as a 2-step process.
