What is an NDIS Continence Management Plan?
A Continence Management Plan is a detailed, clinical document created by a qualified Continence Nurse Specialist. It outlines how to manage a Participant’s continence care safely and effectively each day. This includes recommendations for continence products, step-by-step care routines, and practical strategies tailored to the individual’s goals, preferences, and living situation.
It’s designed to help Participants manage their continence needs effectively, while also supporting carers and support workers with clear guidance.
Do I need an assessment before receiving a management plan?
Yes. A Participant’s management plan is formed in conjuction with a comprehensive continence assessment. Reassessments are commonly conducted every 12-24 months or if a Participant’s needs change.

Who uses the continence management plan?

Is the Management plan NDIS-compliant?
Yes. Every Caremmunity report is written to meet NDIS requirements, using language and formatting suitable for inclusion in NDIS plan reviews, funding requests, and progress reports.

What’s the difference between a continence assessment and a management plan?
A Continence Assessment is a nurse-led clinical evaluation with the Participant or their representative that investigates bladder and bowel function, medical history, and lifestyle factors. This assessment helps identify continence needs and goals.
A Management Plan is the written report that outlines how to manage those needs day-to-day. It includes routines, product recommendations, costings and care strategies.
An assessment must occur for the Management plan and report to be written. Think of it as a 2-step process.
How long does it take to receive the plan?
Once we have all the necessary information—we aim to deliver the completed plan within 10 business days.
If the request is urgent or tied to an upcoming NDIS review, we’ll do our best to prioritise it to meet the required timeline.
